   Proposal Description:
   Aims:   (1)To investigate whether severity of hypertension and its pharmacological treatment might affect the odds of cognitive impairment, and   (2) whether such hypertension effects might be stronger in persons who were ApoE4 positive due to beta-amyloid deposition and cerebrovascular dysfunction.    Hypotheses:   (1)It is hypothesized that increased hypertension severity (untreated hypertension, treated but not controlled hypertension) will increase the odds of MCI and Dementia. It is further hypothesized that participants with controlled hypertension will have increased odds of MCI/Dementia compared to normotensive individuals.   (2) It is further hypothesized that ApoE4 positivity will moderate the relationships between hypertension/treatment status on cognitive outcomes.     Methods: Utilizing a subset of baseline data from the NACC database, participants for these analyses will be 18,906 cognitively-diverse older adults aged 60-109, 58.4 female. Participants will be classified into four groups: untreated normotensive, untreated hypertensive, treated hypertensive, and controlled hypertensive. Multinomial logistic regression will be used with cognitive status as the dependent variable (cognitively normal, mild cognitive impairment, dementia). All analyses will be adjusted for sex, age, and ApoE4 positivity. ApoE4 will also be allowed to moderate hypertension effects.
